Information clause regarding personal data of persons applying for cooperation with HorizOn Business HUB Sp. z o.o. in the field of diagnostic screening tests with the use of DAT technology.

1. The administrator of personal data processed in connection with the implementation of export services provided under the project entitled „Continuation of the foreign expansion of HORIZON Business HUB Sp. Z o.o. on new target markets” co-financed by Toruńska Agencja Rozwoju Regionalnego S.A. from the Export Fund program – grants for SMEs from the Kuyavian-Pomeranian Voivodeship, co-financed under the Regional Operational Program of the Kujawsko-Pomorskie Voivodeship for 2014-2020 is HorizOn Business HUB, a limited liability company based in Lublin, with its registered office at: ul. Graniczna 19, 20-010 Lublin, entered into the Register of Entrepreneurs of the National Court Register under the number KRS 0000533029, NIP PL7773243961 and REGON 360217083, having a local unit in Bydgoszcz at ul. 2. Your personal data is processed in accordance with generally applicable laws on the territory of the Republic of Poland, including in particular Regulation of the European Parliament and of the Council (EU) No. 2016/679 of 27 April 2016 on the protection of individuals with regard to processing personal data and on the free movement of such data and the repeal of Directive 95/46 / EC (general data protection regulation) and in accordance with the Act of 10 May 2018 on the protection of personal data.
3. The rules for the processing of personal data as part of HorizOn Business HUB limited liability company are contained in the Privacy Policy of HorizOn Business HUB limited liability company available at: https://direct-astigmatic-test.com/gdpr-uk/
4. By applying to cooperation with HorizOn Business HUB limited liability company, you give consent to process your personal data in connection with implementation of the Project. Application may cover not only your personal data, but also other natural persons, thus you must acquire their consent for transmitting their data to us. In connection with the Project, the following data are processed:
   • Name and surname
   • Living address
   • Telephone number
   • E-mail address
   • Nationality (including the fact of having or not having Polish nationality)
5. These data are processed on the basis of your consent for the purposes necessary to create and update a database of artifacts of selected civilization diseases disclosed in the DATT App for mobile devices. The data can also be used to offer you / you other activities carried out by HorizOn Business HUB limited liability company, which may allow access to your / your panel in order to increase access to selected functionalities of the application for mobile devices DATT App and the possibility of creating individual reports analytical.
   The data is processed on the basis of the consent granted by you / you pertain to all information obtained through the limited liability company provided by HorizOn Business HUB, the following services:
   • Service 1 – Astigmatism diagnostics service using a DAT remote calibration device with an audio track
   • Service 2 – The service of testing civilization diseases and screening diagnostics with the use of microDAT technology and DAT device
   • Service 3 – Access to the DAT App via the AppStore & Google Play Store
6. Your data will be processed for the period from the date of their receipt by HorizOn Business HUB spółka z ograniczoną odpowiedzialnością until the last day of the expiry of the period of 25 years from the date of their transfer. HorizOn Business HUB limited liability company is obliged to ensure the storage of all documentation obtained for its own purposes and related to the Project, co-financed by Toruńska Agencja Rozwoju Regionalnego S.A. from the Export Fund program – grants for SMEs from the Kujawsko-Pomorskie Voivodeship, co-financed under the Regional Operational Program of the Kujawsko-Pomorskie Voivodeship for 2014-2020 in the above-mentioned period. This period may be extended at the request of HorizOn Business HUB spółka z ograniczoną odpowiedzialnością due to external factors beyond the Company’s control (e.g. legislative changes).
7. Your data may be used only by HorizOn Business HUB spółka z ograniczoną odpowiedzialnością and transferred only to persons related to the implementation of the Project, as well as to persons who supervise the correctness of the Project implementation by HorizOn Business HUB limited liability company on behalf of Toruńska Agencja Rozwoju Regionalnego SA, which provided Grant HorizOn Business HUB limited liability company for the implementation of the Project, if necessary. Access to this data is also possible for persons controlling and auditing the Export Fund – grants for SMEs from the Kujawsko-Pomorskie Voivodeship, co-financed under the Regional Operational Program of the Kujawsko-Pomorskie Voivodeship for 2014-2020. Persons acting on behalf of HorizOn Business HUB spółka z ograniczoną odpowiedzialnością, in particular the Management Board of the Company and all employees, have been obliged by the Company to keep all information regarding personal data confidential and process it in accordance with applicable regulations.
8. You have all the rights provided for by the generally applicable law on the territory of the Republic of Poland. In the case of citizens of the European Union, this means, inter alia, the right to access your data and receive a copy of it, the right to rectify (correct) and receive a copy of it, the right to withdraw consent to the processing of your personal data, the right to delete personal data or limit its processing in situations provided for by law, the right to object to processing data, the right to transfer data and the right to lodge a complaint to the President of the Office for Personal Data Protection.
9. Withdrawal of consent to the processing of personal data will prevent you / you from accessing the functionality of the DAT App and the artifact database maintained by HorizOn Business HUB limited liability company.
10. Due to the tools used by HorizOn Business HUB, in particular IT systems, personal data may be transferred to third countries not belonging to the European Economic Area.
11. Your personal data will not be subject to automated decision making, including profiling.
